<br /> the employee shall report to work if released from jury service prior to 5 :00 <br /> P.M. and does not have to report for jury service the following day. An <br /> employee required to serve as a juror shall not have his/her regular starting or <br /> quitting time changed as a result of being called for jury service. <br />18.8 Maternity Leave of Absence Without Pay <br /> 18.8.1 Maternity leave of absence without payor benefits shall be granted <br /> upon request to non-disabled probationary and permanent female <br /> Fire Fighters for that period oftime requested, up to one (1) year. <br /> 18.8.2 Maternity leave shall be granted when the following conditions have <br /> been met: <br /> 18.8.2.1 The Fire Fighter shall notify her immediate <br /> supervisor in writing, accompanied by her physician's <br /> certificate of pregnancy, as soon as possible after <br /> pregnancy has definitely been determined, but no <br /> later than ninety (90) days prior to the tentative date <br /> on which the leave is to begin. Such notice shall <br /> include the tentative dates on which the leave shall <br /> begin and end. <br /> 18.8.2.2 Within thirty (30) days of the beginning of the <br /> maternity leave, the Fire Fighter shall submit to the <br /> Human Resources Director the specific date she <br /> intends to begin the leave, accompanied by her <br /> physician's written statement attesting to the Fire <br /> Fighter's ability to continue performing the full <br /> schedule of her duties and responsibilities. She shall <br /> continue on active duty until the specific date, <br /> providing she performs the full duties and <br /> responsibilities of her position and furnishes <br /> additional health statements form her physician upon <br /> reasonable request. <br /> 18.8.2.3 Prior to the establishment of a specific date for return <br /> to duty, the Fire Fighter shall submit to the Human <br /> Resources Director a notice of intention to return to <br /> duty, accompanied by her physician's statement <br /> certifying that the Fire Fighter is medically qualified <br /> to assume regular duties and responsibilities. <br /> 36 <br />
