size of pipe, type of pipe, shape of pipe, pipe joint length, total pipe length, actual pipe length <br />surveyed , pre -cleaning method use, weather condition and the overall pipe rating condition <br />of the pipe segments (from MH to MH) indicating the following ratings 1 - if the pipe is in <br />excellent condition, 2 if the pipe is in good condition, 3 - if the pipe is in fair condition, 4 — if <br />the pipe is in poor condition and 5 - if the pipe is in bad condition and needing immediate <br />removal and replacement. <br />3. Submit a GIS with color coded sewer basin site map and a hard copy and electronic copy <br />(PDF) map with the segments color coded by hand showing pipe segments (from manhole to <br />manhole) pipe condition rate designation in color coding as follows; Pipe segments (from <br />manhole to manhole) with 1 - excellent pipe rating condition in blue color; pipe segments <br />with 2- good pipe rating condition in green color; pipe segments with 3 -fair pipe rating <br />condition in yellow color; pipe segments with 4 - poor pipe rating condition in purple color <br />and pipe segments with 5 bad pipe rating condition in red color. <br />DEADLINE: <br />CONTRACTOR shall complete all work within One Hundred Eighty (180) calendar days of the <br />City's notice to proceed. CONTRACTOR shall put an additional crew on this project if needed <br />to maintain the schedule at no additional cost to the City. CONTRACTOR shall average <br />approximately 3,000 linear feet per day, per crew, in order to allow CONTRACTOR to finish <br />within the City's timeframe. CONTRACTOR shall provide the City with a detailed work <br />schedule, to include streets and dates, at City's request. <br />PAYMENT TERMS: <br />CONTRACTOR shall be said on the basis of actual auantity of work completed in linear <br />feet of sanitary sewer pipe serviced, CCTV and cleaned, and not to exceed $344,355.90. <br />The fee, per foot of sanitary sewer pipe serviced, shall include all labor, materials, and <br />equipment necessary to clean and inspect manholes, clean and inspect sewer lines, implementing <br />traffic control, procuring water, disposing of debris, and delivering hard drive as noted. <br />REV: 08-04-17 PR <br />Page ] 3 of 14 <br />ATTY/AGR.2017.173/National Plant Services, Inc. <br />
