Revisions/updates to the LMP and NPDES General <br />Permit water quality monitoring plan will also be provided as needed. <br /> <br />INSURANCE <br /> <br />Cressey & Associates will provide the City with a Certificate of Liability Insurance <br />naming the City as Additional Insured. The attached Table 1 provides a detailed break down of labor and expense <br />costs. Scott Cressey's billing rate for this contract is $90 per hour through June 30, 2004 <br />and $100 per hour from July 1,2004 through June 30, 2005. Services provided will be <br />billed on a time and materials basis. Billings will be monthly with invoices due and <br />payable upon receipt and overdue after 45 days.
