Eghibit A Page A- 2 <br /> PROGRAM DESCRIPTION <br /> b. Emergency Food - coordinate donations to stock food pantry and distribute <br /> approximately 450 bags of food. <br /> c. Emergency Shelter - provide motel voucher assistance to approximately 30 <br /> families/individuals. <br /> d. Employment Services - develop volunteer group to provide intake and job <br /> information to 150 persons. <br /> e. Advocacy - to assist a minimum of 350 clients to represent their interests to <br /> governmental agencies, and other institutions. <br /> f. Financial Assessment-provide simple budgeting information to at least 75 clients. <br /> g. Translation -provide in-house translation assistance to 400 clients. <br /> h. Form Assistance - recruit volunteers and provide assistance in filling out forms <br /> (County TANF, Medi-Ca1, Food Stamps, Child Support, State Disability, <br /> Unemployment, etc.)to approximately 400 clients. <br /> i. Crisis Intervention-provide immediate crisis intervention service for approximately <br /> 75 clients. <br /> Effects of Services: Services will a11ow low income residents in crisis to stabilize their <br /> situations over the short-term and receive assistance in planning for long-term solutions to <br /> emergency situations. Monolingual Spanish speaking residents of the community will be <br /> provided access to services where language had been a barrier and residents will, in general, <br /> have easy access to information on how to resolve issues of concern. <br /> II. OUTCOMES: <br /> Contractor will collect and report to County on a quarterly basis data on outcomes of various <br /> program activities: <br /> A. Emergency Services <br /> For Emergency Assistance (Item 9 of Direct Contract Services Provided in Quarterly/ <br /> Annual Report including: clothing, food, shelter vouchers, transportation, and other) <br /> delivery of the service itself will be the outcome. <br /> B.
