However, Delta will not provide Benefits <br />for any employee or his or her dependents unless (1) the employee is <br />included on the list of Eligible Employees submitted as required by this <br />Article (or any revision or correction of such a list), and (2) the appropriate <br />payment is made as required by Article 3 of this Contract, for the months <br />in which Delta provides covered dental services. <br /> <br />City of Redwood City agrees to enroll all of its "Eligible Persons" in this <br />Program. All employees of City of Redwood City meeting the eligibility <br />requirements of this Article are "Eligible Employees" under this program <br />unless City of Redwood City offers one or more alternate programs of <br />dental coverage. In that event, Eligible Employees will continue to be <br />eligible under this program unless they file a choice card with City of <br />Redwood City electing an alternate program during an open enrollment <br />period agreed upon between Delta and City of Redwood City. <br /> <br />On or prior to the first day of every month, commencing on February 1, <br />1998, City of Redwood City will compile and furnish Delta with a list of <br />all Eligible Employees, showing their federally-assigned Social Security <br />numbers, the dates of hire, and, if applicable, the location code. City of <br />Redwood City also agrees to include in the list all persons electing <br />continued coverage under Article 10, showing their federally-assigned <br />Social Security numbers and dates of election. <br /> <br />Employees and their dependents are not eligible during a period the <br />employee does not report to work on a regular basis and is not actively <br />employed as determined by City of Redwood City. Eligibility resumes on <br />the first day of the month following the return to active employment if <br />amounts due to Delta for Eligible Persons have been paid. But, eligibility <br />can continue without interruption if City of Redwood City continues to <br />report the employee as an Eligible Employee and the amounts due to Delta <br />are paid on the employee's behalf. <br /> <br />An employee absent from work due to a leave of absence governed by the <br />"Family and Medical Leave Act of 1993," (P.L. 103-3) will not be subject <br />to Section 2.9. <br /> <br />6 <br /> <br />,.- ,~-~...._.. .-.,----.-..---...-.-.-------- <br />
