Output shall <br />confonn to the RFP dated May 19, 1998. That RFP is herein incorporated by reference. <br /> <br />Two levels of service shall be provided, detennined by whether the application is for tree removal or <br />tree pruning. <br /> <br />For each tree inspected during an on-site examination the following shall be provided to the City:: <br /> <br />1. <br /> <br />Tree Removal Pennit Applications: <br />1. "Site Observation Checklist" recording the tree's species, size (ordinance diameter <br />between 6" and 36" above grade), tree and site conditions pertinent to the subject <br />application, <br />2. Photograph of the tree (one PolaroidTM snapshot), <br />3. Roughly hand sketched "not to scale"site map, <br />4. Written "Inspection Report" with recommendations to the City for approving or <br />denying removal, stating the basis for such recommendation. <br />5. Recommendation that the applicant plant a replacement tree when appropriate in <br />pennit conditions and property spacing. <br /> <br />II. <br /> <br />Tree Pruning Pennit Applications: <br />1. Written "Inspection Report" recording the tree's species, size (ordinance diameter <br />between 6" and 36" above grade), and basic tree and site conditions pertinent to the <br />subject application. Include recommendations to the City for approving or denying <br />the proposed pruning, stating the basis for such recommendation, <br />2. Provide recommendations for performing the pruning task within ISA Pruning <br />Guidelines. <br />3. Ifrecommending denial, or where greater detail would be important, "Site <br />Observation Checklist" similar to the material provided above for removals (§I, <br />1.), <br />4. If recommending denial, or where greater detail would be important, one <br />PolaroidTM photograph of the tree, <br />5. Roughly hand sketched "not to scale"site map, if it would reduce ambiguity or <br />provided needed detail. <br /> <br />Ray Morneau shall personally make the on-site inspections and write the inspection reports. <br /> <br />..--,.----.,.,.-...,...,.., <br />
