<br />.5./e-z' <br /> <br />City Council approved an agreement with MHN on January 5, 1998. As the agreement <br />allowed for extensions, staff has been extending the agreement through amendments to this <br />agreement, annually. In January 2003, MHN issued a new agreement incorporating all <br />previous amendments and contract changes including a new life management services <br />program. Recently, the City Attorney's office advised staff that any agreements including <br />extensions and/or amendments must be approved by Council and cannot be approved by <br />staff, as has been our understanding, thus we are requesting Council approval of this <br />amendment and past extensions. <br /> <br />Fiscal Impact <br />The estimated cost to the City will be $21,357 for each of the plan years 05/06 and 06/07. <br />Funds for this program have been allocated in the Workers' Compensation budget. <br /> <br />Alternative <br />Council could direct staff to enter into a services agreement with one of the other vendors that <br />submitted a proposal. <br /> <br />~~'\~IIWlD- &~Q <br />Lisa Flowers-Ford <br />Employee Benefits Coordinator <br /> <br /> <br />~ <br /> <br /> <br />Ed Everett <br />City Manager <br /> <br />Attachments <br />A.
