<br />q A \3> <br /> <br />ATTACHMENT # 4. <br /> <br />NON-DISCRETIONARY PERMIT FORM <br /> <br />¡¡f.~~.~~f, <br /> <br /> <br />NON-DISCRETIONARY PERMIT APPLICATION <br />FOR LARGE FAMILY DAY CARE <br /> <br />Applicant: <br /> <br /> <br />Address: <br /> <br />Phone: <br /> <br />DEFINITION <br /> <br /> <br />A large family day care home shall mean a home which provides day care for 7 to 12 <br />children, inclusive, including children under the age of 10 years who reside in the home. <br /> <br /> <br />Pursuant to Article 2 of the Redwood City Zoning Ordinance and to the California Health <br />and Safety Code Section 1596.70, a permit for a large family day care home is approved <br />under the following conditions: <br /> <br />1 ) <br /> <br />The provider has secured a large family day care home license <br />from the State of California, Department of Social Services. <br />(Community Care Ucensing can be contacted at 266-8800). The <br />applicant shall provide proof of such license to the Planning Division <br />within 90 days of approval by the Planning Division. If the license is <br />revoked for any reason, the applicant shall immediately notify the <br />Planning Division. <br /> <br />2) <br /> <br />The facility is the principle residence of the provider and the use is <br />clearly incidental and secondary to the use of the property for <br />residential purposes. <br /> <br />3) <br /> <br />No structural changes are proposed which will alter the character <br />of the single-family or multi-family residence. <br /> <br />4) <br /> <br />Provisions have been made to provide, at a minimum, one off- <br />street parking space per employee of driving age not living in the <br /> <br />» »>~'----">-'T--- <br />
