��' � � <br /> TASK ORDER/ADDITIONAL SERVICES <br /> NO. 4 <br /> � This Task Order is entered into pursuant to the terms and conditions of the <br /> following described agreement (the "Agreement"), entered into by and between the City <br /> of Redwood City, a r�unicipal corporation of the State of California (the "City") and the <br /> � contractor (the "Contractor") hereinafter identified: <br /> Title of Agreement: Professional Consulting Services <br /> Contractor: Subsurface Consultants, Inc. <br /> 3736 Mt. Diablo Blvd., Suite 200 <br /> Lafayette, CA 94549-3659 <br /> Date of Agreement: Sept. 12, 1996 <br /> 1. Task Described. Contractor shall perform the following services, which are in <br /> addition to those services described in the Agreement. <br /> Description of Services: Environmental Site Characterization and report <br /> preparation, Phase One <br /> 2. Compensation. City shall compensate Contractor for the services described in this <br /> Task Order under one of the finro following alternatives, as indicated (check and <br /> complete, as appropriate): <br /> a) (check, if appropriate) Compensation shall be based upon the rates of <br /> charge and method of calculation described in the Agreement. (See Confractor's <br /> Table 1— Fee Estimate, schedule of hourly rates.) <br /> Compensation shall be provided as follows: <br /> Dollar value of Task Order No. 4 not to exceed $80,340.00. <br /> 3. Time of Performance. Contractor shall perform the services to be provided <br /> pursuant to this Task Order on or before the following: <br /> Date of Completion: <br /> Extend date of completion from 8/01/2000 to June 30, 2001. <br /> 4. Approval. In accordance with the Agreement, this Task Order was approved by <br /> City's City Council on December 18, 2000, by motion duly made and carried (copy of <br /> Minute Order evidencing approval attached). <br /> i <br />
