I ß--;LD <br /> <br />EXHIBIT A <br /> <br />CITY OF REDWOOD CITY <br />KAINOS HOME AND TRAINING CENTER <br />ADULT BEGINNINGS HOME <br />FISCAL YEAR 2005/2006 <br /> <br />CONTRACT OBJECTIVES <br /> <br />Quantifiable Goals to be Provided With Performance Indicators <br />Re uested Funds <br />Provide housing to 12 adults with developmental <br />disabilities <br /> <br />All clients will meet 80% of their training goals in <br />functional living skills as set in individual meetings. <br /> <br />Provide each client at the Adult Beginnings Program <br />with three social and recreational activities per week. <br /> <br />10% of clients will move to more independent setting <br />within the rant eriod <br /> <br />Housing remains secure and solvent. <br />This will be measured through our clients' <br />progression in the program, and making <br />sure there is appropriate funding to keep <br />AB program running in a manner to <br />su ort the clients. <br />This will be measured by charting their <br />individually set goals in functional living <br />skills on a weekl basis. <br />All clients attend three social and or <br />recreational activities that they are <br />interested in er month <br />1 person moving t placement of greater <br />inde endence. <br /> <br /># of Unduplicated RWC Residents/Households to be Assisted (Only programs serving <br />individuals may report individuals.
