<br />Ion\iisioo the Way Home <br /> <br />Clara-Mateo Emergency Shelter <br /> <br />The following outcome objectives and interventions are proposed: <br /> <br />. HBJECTIVE #1: At the end of the contract period, 90% of 50 homeless Redwood City <br />I esidents will have immediate, emergency housing and nutrition needs met; <br />. Conduct outreach to 100 homeless individuals and couples living in Redwood City; <br />. Provide 3,000 shelter bed nights to 50 individuals arulUally; . <br />. Operate the emergency shelter 24 hours daily for 365 days per year. <br />. Provide three meals per day to 50 homeless Redwood City residents; <br />. Continue to develop volunteer sponsorships with service organizations that donate <br />meals and with the Food Bank for low-cost food items. <br /> <br />. ! !BJECTIVE #2: At the end of 12 months, at least 55% of shelter residents will exit the <br />I IOgram to a more stable fonn of housing. <br />. Conduct intake and initial assessment of the needs of 50 homeless individuals; <br />. Develop a mutually agreeable, individual case management plan for each of 50 <br />homeless individuals with pennanent housing as a specific goal; <br />. Conduct housing search and employment search if necessary, to identify affordable <br />permanent housing. <br /> <br />A mmber ofEV ALUATION TOOLS are utilized to measure program success: <br />. Intake statistics are maintained by agency staff <br />. Sign-in sheets document shelter usage 365 nights a year <br />. Written case management plans and monitoring notes demonstrate progress toward <br />removing barriers to self-sufficiency. <br />. A customer satisfaction swvey gathers client input into the program. <br />An established grievance procedure is in place to provide an avenue for client concerns that <br />migt t need additional attention. <br />b. How does your proposed program or project help people to achieve self-sufficiency <br />and reduce their need for other governmental assistance? The goal of the CMA <br />Erne 1~ency Shelter program is to end an individual/family's homelessness by preparing them <br />for hi' lependent living through strong, personalized case management and access to resources <br />that' \ ill facilitate achievement of their goals for self-sufficiency. After an initial intake and <br />asse~~ment is completed for each client, the client and the assigned case manager develop an <br />indhidualized service plan. The service plan may include details about the client's housing <br />plan, \~mployment and/or educational/job skiJIs plan, recovery plan, mental health care, medical <br />care, Hnd income and money management. The service plan acts as the blueprint for all client <br />activ 1 y while slhe resides in the program, with a focus on maintaining a stable income, <br />stabi ¡zing physically and mentally, reducing debt, saving money, working on any treatment or <br />subst ,¡nee abuse issues, and preparing for independent living. <br /> <br />c.
