Responsibility: Prepared by Contractor Timing: Not later than August 1 Tasks: ".Preparation of Audited Financial Statements II.Preparation of Management Representation Letter ".Preparation of Expense Projections ".Pre aration of Su ortin Documentation REPORTING OF OPERATIONAL INFORMATION The Contractor will submit Attachment 1 - Operational Data Checklist, which will contain a list of required Operation data to be used during the rate review. Operational information shall be prepared for each category of service, (e.g., solid waste, recyclable materials and plant material) for each local jurisdiction, and for the entire SBWMA service area and attached to the check list. Operational information to be provided includes but is not limited to: .:. MIS Reports consisting of Tonnage collected, processed, disposed, recycled, composted, and used as Alternate Daily Cover (ADC); .:. Number of accounts by frequency and type; .:. Set-out rates; .:. Number of routes; .:. Number and type of vehicles and containers; and, .:. Number of personnel (based on average full-time equivalents). As part of preparing its Application, Contractor is encouraged to discuss with the SBWMA any new operations (e.g., the phasing in of new programs) and how its current operations could be altered as a result. AUDITED FINANCIAL STATEMENTS Annual audited financial statements for the SBWMA service area operations are required under the Agreement with the Contractor for each Rate Year. The audited financial statements provide assurance that the financial records fairly present the financial results of the Contractor' operations and serve as the underlying document for the Application. The Contractor will prepare a reconciling schedule of revenues and expenses and other reports contained in the Application to the audited financial statements.
