Accomplishments In terms such as dates, activities <br /> 2 conducted, quantities of products used, produced or distributed, and numbers of <br /> 3 participants and responses. <br /> 4 <br /> 5 8.03 Reports. <br /> 6 <br /> 7 8.03.1 Report Formats and Schedule. Records shall be maintained in <br /> 8 forms and by methods that facilitate flexible use of data contained in them to structure <br /> 9 reports, as needed. Reports are intended to compile recorded data into useful forms of <br /> 10 information that can be used to, among other things: <br /> 11 <br /> 12 A. Determine and set rates and evaluate the financial efficacy of <br /> 13 operations; <br /> 14 <br /> 15 B. Evaluate past and expected progress towards achieving the <br /> 16 Agency's diversion goals and objectives; <br /> 17 <br /> 18 C. Determine needs for adjustment to programs; and, <br /> 19 evaluate service recipient service and Complaints. <br /> 20 <br /> 21 Contractor may propose report formats that are responsive to the <br /> 22 objectives and audiences for each report. The format of each report shall be approved <br /> 23 by Agency. Contractor agrees to submit all reports on computer discs or by modem in <br /> 24 a format compatible with Agency's software/computers at no additional charge, if <br /> 25 requested by Agency. Contractor will provide a certification statement, under penalty <br /> 26 or perjury, by the responsible Contractor official, that the report being submitted is true <br /> 27 and correct to the best knowledge of such official after their reasonable inquiry. <br /> 28 <br /> 29 Quarterly reports shall be submitted within forty-five (45) calendar <br /> 30 days after the end of the report quarter.
