<br /> 1 <br /> 2 1) The most recent editions of Insurance Services Office form <br /> 3 number GL 0002 covering Comprehensive or Commercial General Liability and <br /> 4 Insurance Services Office form number GL 0404 covering Broad Form Comprehensive <br /> 5 or Commercial General Liability; or Insurance Services ()ffice Commercial General <br /> 6 Liability coverage ("occurrence" form CG 0001). <br /> 7 <br /> 8 2) The most recent editions of Insurance Services ()ffice form <br /> 9 number CA 0001 covering Automobile Liability, code 1 "any auto" and endorsement <br /> 10 CA 0025. <br /> 11 <br /> 12 3) Workers' Compensation insurance as required by the Labor <br /> 13 Code of the State of California and Employers Liability insurance. <br /> 14 <br /> 15 B. Minimum Limits of Insurance. Contractor shall maintain limits no <br /> 16 less than: <br /> 17 <br /> 18 1) Comprehensive General Liability: Ten Million Dollars <br /> 19 ($10,000,000) combined single limit per occurrence for bodily injury, Personal injury and <br /> 20 property damage. <br /> 21 <br /> 22 2) Automobile Liability: Ten Million Dollars ($10,000,000) <br /> 23 combined single limit per accident for bodily injury and property damage. <br /> 24 <br /> 25 3) Workers' Compensation and Employers Liability: Workers' <br /> 26 compensation limits as required by the Labor Code of the State of California and <br /> 27 Employers Liability limits of $1,000,000 per accident. <br /> 28 <br /> 29 C. Deductibles and Self-Insured Retentions. Any deductibles or self- <br /> 30 insured retentions are the responsibility of Contractor and must be declared to Agency.. <br /> 31 At the option of the Agency, either: the insurer shall reduce or eliminate such <br /> 32 deductibles or self-insured retentions as respects the Agency, SBWMA, their officials <br /> 33 and employees, directors, agents and volunteers; or Contractor shall procure a bond <br /> 34 guaranteeing payment of losses and related investigations, claim administration and <br /> 35 defense expenses. <br /> 36 <br /> 37 D. Other Insurance Provisions. The policies are to contain, or be <br /> 38 endorsed to contain, the following provisions: <br /> 39 <br /> 40 1) General Liability and Automobile Liability Coverages <br /> 41 <br /> 42 a) The Agency, SBWMA, their officials, employees, <br /> 43 directors" agents and volunteers are to be covered as additional insureds as respects: <br /> 44 liability arising out of activities performed by or on behalf of Contractor; products and <br /> 45 completed operations of Contractor; Premises owned, leased or used by Contractor; or <br /> Atty j Agrj2005.022 <br /> 060205 -50- <br />
