AGREEMENT FOR WORKERS' COMPENSATION <br /> CLAIMS ADMINISTRATION SERVICES <br /> <br /> This Agreement is made effective this 1st day of July 2002, between Claims <br /> Management, Inc. (hereinafter referred to as "CMl"), and the City of Redwood City <br /> (hereinafter referred to as "City"). <br /> <br /> RECITALS <br /> <br /> WHEREAS, City desires to self-insure its liability to its employees pursuant to the <br /> Workers' Compensation laws of the State of California, and <br /> <br /> WHEREAS, CMl is qualified and willing to administer the self-insured workers' <br /> compensation program undertaken by the City, and <br /> <br /> WHEREAS, City desires to retain the services of CMl and CMl agrees to render <br /> its services for City on the terms and conditions of this Agreement. <br /> <br /> NOW, THEREFORE, in consideration of the mutual promises, conditions and <br /> covenants hereinafter set forth, CMl and City agree as follows: <br /> <br /> I. TERM <br /> <br /> The term of this Agreement shall be from 12:01 a.m., July 1, 2002 to midnight <br />on June 30, 2003, unless sooner terminated as provided in Section IV, hereof. <br /> <br /> II. CONSIDERATION <br /> <br /> The annual fee payable to CMl pursuant to this Agreement shall be $129,000, <br />payable in arrears in monthly installments of $10,750, which payment shall be due on <br />the first day of each month beginning August 1, 2002. <br /> <br />F:A'FrY/AGR/AGR.048 1 <br />061902 <br /> <br /> <br />
