<br />Exhibit A <br /> <br />Page A-I <br /> <br />PROGRAM DESCRIPTION <br /> <br />In consideration of payments herein provided for, Contractor shall, under the general direction of the <br />Director of the Human Services Agency, or her authorized representatives, provide for the following <br />services to benefit the citizens of San Mateo County as described below: <br /> <br />THE CITY OF REDWOOD CITY, FAIR OAKS COMMUNITY CENTER INFORMATION & <br />REFERRAL PROGRAM IS A MUNICIP AL CORPORATION OF THE ST ATE OF CALIFORNIA <br />OPERATING A HUMAN SERVICES ASSISTANCE PROGRAM IN SAN MATEO COUNTY <br />FOR LOW INCOME PEOPLE. <br /> <br />CITY OF REDWOOD CITY, FAIR OAKS COMMUNITY CENTER INFORMATION & <br />REFERRAL PROGRAM WILL HEREIN BE REFERRED TO AS "CONTRACTOR". <br /> <br />Contractor is located at 2600 Middlefield Road, Redwood City, California 94063. Operating hours <br />are Mondays, Wednesdays and Fridays: 8:00 a.m. to 12:00 noon and 1 :00 p.m. to 5:00 p.m.; <br />Tuesdays and Thursdays: 8:00 a.m. to 12:00 noon and 1:00 p.m. to 8:00 p.m.; telephone number is <br />(650) 780-7500 and fax number is (650) 298-8184. <br /> <br />I. <br /> <br />CLIENT SERVICES: <br /> <br />The Contractor will provide the following services, supporting activities and related <br />information from July 1,2000 through June 30,2001: <br /> <br />Services Provided: Contractor serves as the primary access point for information on <br />resources available to low income persons in Spanish and English in the Fair Oaks <br />community; provides needs assessment, crisis intervention, short-term emergency material <br />assistance and translation services. Contractor also serves as a referral site for the County <br />Shelter Network program and intake site for Season of Sharing Assistance for the service <br />area. These services will be provided in an environment that respects the dignity ofthe client <br />and endeavors to foster independence and self-sufficiency. <br /> <br />The following number of clients to be served are estimates only. Contractors may provide <br />services which vary no more than 10% under the estimated number to be served. <br /> <br />1. 4,400 contacts with residents of the County by phone or in the office for purposes of <br />giving information (1,500), providing needs assessment (2,200), and case management <br />(700). <br /> <br />2. Provide direct services to clients in the following areas: <br /> <br />a.
