<br />yards, carnivals, parking lots, and industrial substations <br /> <br />without first obtaining an electrical permit for such work, <br /> <br />(ii) perform any electrical work for the connection of any <br /> <br />electrical system, or electrical work to a supply of <br /> <br />electricity, without first obtaining an electrical permit <br /> <br />(iii) perform electrical work relating <br /> <br />for such work, or, <br /> <br />to outside conductors or electrical facilities exterior to <br /> <br />any building, structure, or premises without first obtaining <br /> <br />an electrical permit for such electrical work. <br /> <br />"(b) <br /> <br />To obtain an electrical permit, an applicant shall <br /> <br />first file an application therefor in writing on a form <br /> <br />Every such application shall: <br /> <br />" (1) <br /> <br />furnished for that purpose. <br /> <br />" (2) <br /> <br />" (3) <br /> <br />" (4) <br /> <br />" (5) <br /> <br />" (6) <br /> <br />" (7) <br /> <br />Identify and describe the electrical work or <br />electrical system to be covered by the permit <br />for which application is made; <br /> <br />Describe the land, building, structure, or <br />premises upon which the proposed electrical <br />work is to be done, with reference to lot, <br />block, tract, and house numbers and street <br />address, or similar description that will <br />readily identify and definitely establish <br />the location of the work; <br /> <br />Indicate the use for which the proposed <br />electrical work or electrical system is <br />intended; <br /> <br />Be accompanied by plans and specifications <br />when required by the Electrical Inspector; <br /> <br />Be signed by the applicant, or his authorized <br />agent, who may be required to submit evidence <br />to indicate such authority; and <br /> <br />Certify compliance to Section 3800, State of <br />California Labor Code regarding Workers' <br />Compensation Insurance. <br /> <br />Give such other information as reasonably may <br />be required by the Electrical Inspector. <br /> <br />-2- <br />
