for such permit in the amount established by resolution adopted pursuant to the provisions of Article IX of this chapter. "Section 27.90. Application. The Building Inspector, before issuing any permit required by the preceding section, shall require a written application to be made and filed, in which application shall be set forth the following: " (a) The name and address of the person making such application. " (b) The location and description in detail of the work to be done and the facilities to be used or maintained. "(c) Any other information deemed necessary by the Building Inspector to determine whether the proposed work or use complies with the provisions of this article and other ordinances of the city regulating the construction of sewage disposal systems on private property. Report of Civil Engineer. Before "Section 27.91. issuing the permit required under Section 27.89, the Building Inspector may require the applicant to furnish a report prepared by a registered, practicing civil engineer. Such report may be required to include such soil tests, percolation tests, geological data, and design of a sewage disposal system based on such data, which indicates that such system will provide adequate capacity and percolation or leaching. "Section 27.92. Issuance of Permit. Upon receipt of the written application required under Section 27.89, payment of all required fees and charges, verification of granting of the variance authorized under Section 27.85, and approval of the application in accordance with the provisions of Section 27.89 through this Section 27.92, the Building Inspector shall issue a written permit to construct or put into use such private sewerage disposal system. -34-
