<br />Historic Courthouse Reconstruction <br />May 3, 2005 <br />Page 2 of 4 <br /> <br />2. Review submittals, shop drawings and samples for conformance with the approved <br />design concept of the Project, and for compliance with the information given in the <br />construction documents. <br />3. FRP Process: Review and evaluate the results of the sculpting of the special <br />reconstruction features required by the project. Include reviews of the final mock-ups <br />prior to fabrication. Maximum of (2) visits to the fabrication facility. <br />4. Review and respond to Requests for Information (RFI's) <br />5. Observe the quality of the work, and endeavor to guard against defects or deficiencies. <br />Such observations will be conducted in conjunction with the periodic GAG meetings. <br />6. Provide a final site visit to establish date of "Substantial Completion". Prepare and issue a <br />punch list specifying the work to be completed and / or corrected before final payment is <br />issued. <br /> <br />Construction Closeout Phase <br />1. Prepare architectural record documents upon receipt of as-built information from general <br />contractor. Changes shall be compiled from a paste-up of field issued information. New <br />CAD work is not included. <br /> <br />ENGINEERING CONSULTATION <br />To be provided per attached proposals: <br />1. Structural: DES <br />2. Electrical & Plumbing: Alfa Tech, Inc. <br /> <br />SCHEDULE <br />Refer to the attached Project Schedule. <br /> <br />ASSUMPTIONS <br />The following assumptions form the basis of this proposal: <br />1. No changes to the previously approved construction documents, which includes city plan <br />check process. <br />2. All DES work, and related responsibilities, will end prior to start of the work on the plaza. <br />3. The plaza work will conform to the reconstruction work, wherever possible. <br />4. Our fee is based on the period of time shown on the Project Schedule. <br /> <br />EXCLUSIONS <br /> <br />The following services are not included in our Basic Services fee, but can be provided on a <br />negotiated fee or time and expense basis per the attached Schedule of Charges. <br />1. Discovered conditions in the field that result in: <br />. Extensive investigation <br />. Modifications to the documents <br />. Changes to the design <br />. Reconciliation of dimensions per existing conditions <br />2. Plaza related work. <br />3. Security and telephone - data design <br />4. Building permit application fees <br />5. Coordination of bidding process, except as included in scope of work <br />6. Artwork selection or placement <br />7. Signage, other than code required signage. <br />8. Removal or assessment of hazardous materials <br />9.
