<br /> 6.18 <br /> Page 12 <br /> EXHIBIT A <br /> Program/Project Description <br /> Differential Response <br /> JANUARY 1, 2006 THROUGH JUNE 30, 2008 <br />Contractor will provide, to the satisfaction of Redwood City 2020 or his designee, <br />services in connection with the specified Differential Response (DR) services, as <br />described in Attachment I, to Path One and Path Two families in Redwood City as <br />follows: <br />I. TraininQ and Standardized Assessments <br />Determine the appropriate staff to attend the training, including but not limited to, <br />Case Managers, Program Director, supervisors, managers and multi-disciplinary <br />team members. <br />II. Initial Assessments and Contact <br />1. Path One - Contractor's Project Manager shall meet with County Community <br />Worker (CCW) through a Multi-Disciplinary Team (MDT) process, to jointly <br />identify families to be served. Program Director shall determine the assignment <br />of referrals to Case Managers. Case Manager will engage family and assess for <br />service needs using FAST. <br />2. For Path Two, County Social Worker (SW) contacts Program Director who <br />assigns the case to a Case Manager. SW and Case Manager conduct an MDT <br />and joint response. The SW will assess for risk using Comprehensive <br />Assessment Tool (CAT). Case Manager will engage family and assess for <br />service needs using FAST. <br />III. Case ManaQement <br />1. Path One Referrals, <br />. Develop service plan and assume all case management duties. <br />. Assess families' services needs using Family Assessment Screening Tool <br /> (FAST). <br />. Track and monitor cases activities through the life of a case and case closure <br /> using the web-based Community Approach to Relating and Engaging with <br /> Families (CARE) tracking system. <br />. Follow DR process as delineated in Attachment I. <br />. Re-refer cases to Child Protective Services (CPS) Hotline, as appropriate. <br />2. Path Two Referrals, <br />. Visit the client in a joint response with SW.
