Administration of County Community Worker <br /> <br />Be responsible for providing administrative services to the CCWs including: <br /> <br />- Approving and signing timecards, leave, sick, etc. <br />- Conducting performance evaluation. <br />- Providing coaching and mentoring. <br />- Identifying other appropriate training. <br /> <br />VI. Multi-Disciplinary Teams (MDTs) <br /> <br />Provide Contractor with policies and procedures related to MOTs. <br /> <br />VII. Service Intearation <br />1. Provide the following resources to serve and support families that are referred for DR <br />services: <br />- CCWs <br />- Psychiatric Social Workers <br />2. Provide Contractor with two (2) computers for start-up. <br /> <br />VIII. Policies and Procedures <br /> <br />Provide the following polices and procedures: <br />- DR <br />- MOTs <br />- FSSTs <br /> <br />IX. Data and Evaluation <br /> <br />1. Gather data and reports using the CARE system. <br />2. Provide Quarterly Customer Service Survey forms to Contractor. <br /> <br />13 <br />
