Exhibit A - Attachment 1
Program Description
Form 5 Agreement # C2805010
Outcomes and Evaluation Plans
Community Program Development Grant
FY 2005-06

Contracting Entity: Ci of Redwood Ci Parks Recreation and Communi Senrices
Name of Proposal: Communi" Inclusion Initiative

A. How will our proposal address and overaD strategy for systemic change.
Inclusion is a concept in which individuals with disabilities are given the opportunity to make
their own choices about the types of leisure activities they wish to engage in. While
specialized programs are available in San Mateo County» there are currently no local
government parks and recreation programs initiating and/or promoting full-inclusion.
Redwood City will be breaking the mold with this pilot model. The Community Inclusion
Initiative will address a major need for more recreation resources for those with disabilities in
our county, will provide education to community leaders and recreation professionals
regarding inclusion and, in addition, will incorporate community involvement through
volunteerism. With the success of this program, Redwood City plans to implement a strategy
for systemic change by promoting our pilot program to other cities and agencies. If each city
on the peninsula were to initiate an Inclusion Program, the issues regarding lack of recreation
services for people with disabilities would virtually be eliminated. San Mateo County
residents would be made more aware of the challenges and successes of people with
disabilities as they enjoy recreation programming side by side. Residents. especially our youth
community. will be able to make lasting relationships and partnerships with a person with
disabilities as they ultimately mentor each other. OveraJl, San Mateo County will be become
more open-nùnded, diverse and accepting of those with varying abilities.

B. How will this program be monitored and evaluated?
This program will be monitored through tWo systems. The first system will be an analytical
look at the number of participants with disabilities enrolled in regular recreation classes. The
ultimate goal is to get more people involved in mainstream recreation, and to measure this
result, we will be using SAFARI Recreation Ware. This program specializes in analyzing and
sorting data based on information inputted when a person enrolls in a program. When a
person with a disability enrolls in a program, they will check off a box indicating that they are
a participant in the inclusion program. To start the program, we will specifically recruit 30
interested participants and guide them through the program. After that, we expect aggressive
advertisinS marketing and word of mouth to assist us in promoting and recruiting participants
to the program.
The second system will be a survey I evaluation process for everyone involved. A survey will
be distributed to all volunteer mentors, participants, family members and group home
managers closely associated with the participant, instructors and agency partners at the end of
the first year to collect feedback on their thoughts, challenges and suggestions for the
program. For participant's who may need assistance with completing this survey, a staff
member will interview them via phone or face to face to get their feedback. What we expect
to learn from the surveys are the strong points and weak points of the program, if the program
is appreciated and analyze potential areas where we can improve.
