<br />Program Description <br />Page 16 of 17 <br />Agreement # C2805010 <br /> <br />C. List the total number of individuals with developmental disabilities that wiD be <br />served by this project. <br />Our estimates suggest that with our Community Inclusion Partnership Consortium, we will <br />have the capabilities to market this program to approx. 4500 potential participants in San <br />Mateo County. This number was reached by adding the total number of clients served by the <br />Golden Gate Regional Center. PARCA, KAINOS and Redwood City's Accessible Recreation <br />Activities Program. We expect to actively serve 30 participants during our pilot program. <br />After necessary training is established and the system overall flows smoothly, we will have the <br />capacity to serve people with disabilities just as we serve our mainstream recreation <br />participants. Depending on class interest, some programs may fill up. but a waiting list would <br />be established on a first come, first serve basis. <br /> <br />D. List of Measurable Outcomes <br />- Increase Inclusion in local parks and recreation programs <br />a) Determine by reviewing 8AF ARI Enrollment Data <br />- Increase personal recreation choices for people with disabilities <br />a) Determine by asking participant's comfort with their choices on the end <br />of the year survey <br />- Increase independence for people wI disabilities pursuing leisure activities <br />a) Determine by asking family members & partnering agencies in an end of <br />the year survey if they believe this goal was met for the community. <br />Increase people with disabilities self-esteem & social skills through mentoring <br />a) Determine by asking participants to self-evaluate their benefits of inclusion, in <br />addition to surveying close family members, group home managers and other <br />individuals who consistently observe the participant. Request for them to <br />document any observed change in personality. <br />Provide rewarding volunteer opportunities for the general public <br />a) Survey volunteers after the complete their mentoring project or once every <br />three months during the pilot year to evaluate their feelings about their role in <br />the program. Additionally, document the number of volunteers active and <br />compare it to previous numbers to determine if more people are volunteering <br />with this program. <br />Provide as a model for other cities and agencies who wish to pursue full-inclusion <br />a) After one year pilot program, send out marketing materials to other cities and <br />agencies that might have an interest in starting the program. Determine if model is <br />successful by the number of agencies that wish to follow our plan. <br />Create stronger alliances and partnerships with neighboring cities and agencies <br />a) Prior to starting this project, we will survey neighboring cities and agencies to <br />evaluate how they perceive information, referrals and relations with each other. At <br />the end of one year, we will re-send the survey to see if they perceive infonnation <br />to be more readily available, have seen more successful referrals and overall have a <br />better relationship with other professionals in the field of Recreation. <br />
