Identify the source for continuation funding that will be available when the period of the grant <br />funding expires. If your agency is going to be the continuation of funding source, please list. <br />State how the proposal/project will continue. <br />Grant will supplement the cost of a one time initial training model for key staff and <br />agencies. Trained professionals will then be able to train othen with no cost from the <br />program. Marketing funds will be supplemented by the increase of enrollment in <br />recreation programs from the success of the program. The more people involved in the <br />program, the more self-sustaining the program will be. The City of Redwood City will <br />continue to fund for key staff positions, office space, equipment and aD other items involved <br />in the program from their operating expense budget. <br /> <br />2. Describe whether the continuation of funding source is ftom a government or a private <br />agency. Provide written verification ftom that agency for the continuation of funds. <br />Government Agency: See Attached Letter of Funding Continuation on page- <br /> <br />3. If a Regional Center is identified as a continuation of funding source, a vendor code must be. <br />provided along with the rate per visit (hourly, daily, monthly, etc.) as set by the Department of <br />Developmental Services' Rates and Vendorization Section. <br />N/A <br />
