Exhibit "A" <br />SCOPE OF SERVICES AND FEE <br />Scoae of Work <br />Contractor will provide management of the City's Galaxy Access Control System and Schlage <br />Quad Primus Master Key System, and provide on-call lock, door hardware, and locksmith <br />services. <br />Task 1: Galaxy Access Control System <br />Contractor shall provide annual management of the Galaxy Access Control System. Contractor <br />shall provide: <br />1. Yearly software maintenance registration; <br />2. Labor to install the latest version of the software, with the City's IT Department's <br />assistance when necessary; <br />3. Yearly inspection of all back-up batteries and replacement when necessary; <br />4. Yearly holiday programming; <br />5. Up to 120 automatic door lock/unlock schedule changes; <br />6. Up to 120 early lockdown of doors that are on a schedule; <br />7. Up to 120 remote unlocking of doors as requested by authorized personnel; <br />8. Up to 300 personnel fob issuances; <br />9. Up to 300 ID card activations; <br />10. Up to 300 deactivation of ID fobs or cards; <br />11. Up to 240 personnel schedule changes; <br />12. Up to 120 activity history reports from the Galaxy System emailed to the requesting <br />party; <br />13. Up to 100 battery backup batteries; <br />14. Up to 300 proximity fobs (HID ProxKey p/n 1346 LNSMN); <br />15. Up to 300 proximity printable badge cards (HID IsoProx card p/n 1386LGGMN); and <br />16. Up to 1,000 Sitemaster signature cards. <br />Task 2: Schlage Quad Primus Master Key System <br />Contractor shall provide annual management of the Schlage Quad Primus Master Key System. <br />Contractor shall provide: <br />1. All labor associated with Sitemaster data entry to keep the key system current with any <br />and all changes within the City's masterkey system; <br />2. Up to 300 Schlage Quad key issuances; <br />3. Up to 300 replacement of worn or broken Schlage Quad keys; <br />4. Up to 120 reports on personnel key holdings; <br />5. Up to 140 Schlage Quad key blanks; and <br />6. Up to 1,000 Sitemaster signature cards. <br />Task 3: On -Call Services <br />Contractor shall provide mobile locksmith services twenty-four (24) hours per day, three hundred <br />sixty-five (365) days per year for all City -owned buildings and sites. Contractor shall maintain a <br />call center/dispatch service that is available twenty-four (24) hours per day, three hundred sixty- <br />five (365) days per year to dispatch a technician to a site within sixty (60) minutes of a call placed <br />by the City for emergencies, and within three (3) hours of a call placed by the City for non - <br />emergencies. <br />REV: 09-03-2020 MI <br />ATTY/AGR.Amend No. 1/1-ockworks Unlimited, Inc.
