6.13. - Page 83 of 366 <br />Employees discharged for disciplinary reasons shall not be eligible for payment <br />of unused sick leave. <br />16.1.8 If an employee's illness or the illness of a family member results in an absence <br />from work for three (3) or more days, a verified statement from a licensed <br />medical practitionerd-^^{^�'s ^^difo^^'^ or other reasonable proof of illness may be <br />required by the City. <br />16.1.9 If a supervisor believes there is an abuse of sick leave, the supervisor may <br />require the employee to provide medical verification for all future sick leave <br />absences for a period of ninety (90) days, upon written notification by the <br />supervisor to the employee, <br />16.2 Industrial Disability Leave <br />161.1 A COA unit employee who suffers any disability arising out of and in the course <br />of his or her employment as defined by the Workers' Compensation laws of the <br />State of California, shall be entitled to industrial disability leave without loss of <br />compensation for the period of leave necessary to recover from such industrial <br />disability,up to a maximum of one (1) year, or upon retirement, whichever comes <br />first. <br />46--97 <br />16.3 Military Leave <br />The provisions of the Uniformed Services Employment and Reemployment Rights Act <br />(USERRA) and Military and Veterans Code of the State of California shall govern the <br />granting of military leaves of absence and the rights of employees returning from such <br />leaves. <br />16.4 Leave of Absence Without Pay <br />16.4.1 Upon written request of an employee, the Personnel Officer may grant a leave of <br />absence without pay for a period not to exceed one (1) year. Any authorization <br />for a leave of absence without pay shall be made in writing by the Personnel <br />Officer and the Fire Chief. <br />16.4.2 During an approved leave of absence, the employee shall not engage in gainful <br />employment unless authorized to do so by written permission of the City. The <br />City may cancel the leave of absence or terminate any employee who violates <br />the terms and conditions of the written permission for the leave. <br />16.5 Jury Duty <br />Any employee whose name shall be selected from the list of trial jurors or serve as a juror in <br />a civil or criminal action pending in a Superior, Municipal, or Justice Court of the State of <br />California, or any Federal court convening in the State of California, or any employee <br />34 <br />102 <br />
