6.B. - Page 185 of 366 <br />10.1.1 The City will contract with the California Public Employee's Retirement <br />System (CaIPERS) for participation under the Public Employees <br />Medical and Hospital Care Act (Government Code Section 22751, et, <br />seq.), for the purpose of providing medical insurance benefits for <br />employees. <br />Upon the request of the City, the parties will re -open this Article 10.1.1 <br />to meet and confer on modifying 10.1.1 to provide a different group <br />health plan. It is understood that any such change is subject to approval <br />of the P.O.A. membership. <br />10.1.2 The City's maximum, monthly contribution for each eligible, active <br />employee for a Health Benefit Plan (as referenced in Government Code <br />22892) shall not exceed the Ca1PERS minimum contribution, adjusted <br />annually by the PERS Board to reflect any change to the medical care <br />component of the Consumer Price Index ara sha4' be founded to the <br />neares! dollar. <br />10.2 Cafeteria Plan <br />10.2.1 Available Benefits: The City shall establishmaintain, in accordance <br />with Section 125 of the IRS Code, a Cafeteria Plan for the purpose of <br />providing employees with access to various health and welfare benefits. <br />Benefits available through the Cafeteria Plan include, but are not limited <br />to: <br />A. Group Health Plan Medical Insurance <br />B. Flexible Spending Account for Dependent Care <br />C. Flexible Spending Account for Medical Expenses <br />10.2.2 City Contribution: Effective January 1, 2016, for each active, full-time <br />employee enrolled in a City -provided medical plan, the City will <br />provide, on a monthly basis, for the purpose of providing minimum <br />essential coverage to employees and dependents, a Cafeteria Plan <br />Allowance equal to the cost of ninety-five percent (95%) of the premium <br />for the employee's selected plan and level of coverage, up to ninety-five <br />percent (95%) of the cost of the Kaiser Bay Area three (3) party <br />premium per month per employee minus any costs incurred by the City <br />to maintain said Cafeteria Plan in compliance with IRS Code Section <br />125, and minus any costs incurred by the City to maintain the Group <br />Health Benefits Plans in compliance with Government code Section <br />22750, et. seq. Such costs include, but are not limited to, surcharges <br />and/or administrative fees. <br />Effective January 1, 2017, for each active, full-time employee enrolled <br />in a City -provided medical plan, the City will provide, on a monthly <br />basis, for the purpose of providing minimum essential coverage to <br />204 <br />
