6.B. - Page 190 of 366 <br />as possible to the benefits in effect on November 1, 1980, without additional cost <br />to the City or the employees, the City may substitute new insurance carriers. The <br />Association will be given an opportunity to review the coverage afforded under <br />such substitute plans before they are implemented. <br />The City and RCPOA agree that in the event state or federal law changes to the <br />extent that mandates are placed on the City or the level of benefits changes during <br />the term of this MOU, the parties agree to reopen the MOU to meet and confer <br />over such changes or mandates, if either party requests to do so. <br />10.4 Dental <br />The City shall continue to provide to eligible employees and dependents fully <br />paid dental insurance through Delta Dental Fee for Service program or through a <br />mutually agreed upon alternative program through the term of this Agreement to <br />include coverage as follows: two thousand dollars ($2,000.00) annual cap for <br />basic coverage and two thousand five hundred ($2,500.00) lifetime cap for <br />orthodonture. Effective January 1, 2016, the City's contribution toward the plan <br />shall be ninety percent (90%) of the plan premium. <br />10.5 Life Insurance <br />The City shall continue to offer to eligible employees the life insurance program <br />in effect on February 1, 1974. Effective September 1, 2011, the City shall provide <br />employees Basic Life Insurance coverage of fifty thousand dollars ($50,000), and <br />Accidental Death and Dismemberment (ADD) coverage of fifty thousand dollars <br />x$50,000), with the City paying one hundred percent 000%,) of the premium. <br />10.6 Lone Term Disability Insurance <br />Eff „tiye .;th the first fid p pefiedafter the .,,7.,ption by the City G,.tineil of <br />the Memer-afidum of Understanding between the POA and the City of Redweed <br />Qt-y-,-41he City will reimburse the Association twenty-four dollars and fifty cents <br />($24.50) per month, per employee towards the cost of Long Term Disability. <br />In the event the cost of the current Long Term Disability plan increases ---during <br />the term of this Memorandum of Understanding, the City will reimburse the <br />actual premium cost up to a maximum of twenty five dollars ($25.00) per month <br />per employee. <br />10.7 Vision Care <br />The City will contract with Vision Service Plan (VSP) or comparable vision care <br />provider to provide vision care benefits for employees and their dependents. The <br />Vision Service Plan B provides for an exam every twelve 121 months, lenses <br />every twelve 121 months if needed, and frames every two 2 years if needed. <br />There is no deductible for employees, but a twenty dollar ($20.00) per person <br />deductible will apply to dependents each time benefits are available and will be <br />209 <br />
