EXHIBIT A <br />1. Scone of Work <br />Contractor will provide various sidewalk repair services for the City of Redwood City as <br />directed. <br />§ Contractor will perform all work so that it complies with City Engineering Standards. <br />§ Contractor will contact U.S.A. North 811 to locate underground utilities prior to starting <br />work in all locations. <br />§ City will inspect work at various stages of each project. <br />§ Prior to fmal inspection and acceptance, Contractor shall restore to original condition any <br />damage to existing structures, facilities, utilities, trees, lawns, shrubs, pavements, or other <br />property caused by Contractor at Contractor's expense. <br />§ City staff will direct Contractor for tree root pruning and tree root removal deemed <br />necessary. <br />§ Contractor will perform all work during the hours of 7:30 am. to 4:00 p.m. Monday <br />through Friday, except on those holidays designated by the City. <br />§ Work done shall not exceed $55,000. <br />Invoicine <br />Contractor will submit invoices on a monthly basis. Invoices must have the following <br />information: an original invoice number, date, and details of work done that include: <br />• Site address, <br />• Item(s) completed per the payment terms below, <br />• Unit measurements, <br />• Unit price, and, <br />• Total price. <br />Contractor will list each address on a separate line within the invoice and will not group <br />addresses together. Invoices shall only include work that is complete and has passed final <br />inspection by the City project manager. <br />2. Payment Terms <br />Contractor shall provide the services outlined above on a per unit basis not to exceed $55,000. <br />Prices listed below are inclusive of all expenses, and include labor, fuel, equipment, tools, <br />notification, materials, disposal of materials and debris, traffic control, tree root work and site <br />cleanup and restoration.
