Description of Services and Recurring Monthly Service Charges: <br />1 Gbps / 1 Gbps Dedicated Internet Access Service provisioned w/ <br />$1,500.00 <br />10 Gbps port' <br />[Included] <br />IP Addresses: 76.14.34.192.0/26 (pre-existing — already in use by <br />[Included] <br />Customer) <br />IP Addresses: 76.14.0.128.0/26 (pre-existing — already in use by <br />Customer) <br />Total Recurring Monthly Service Charges: <br />$1,500.00 <br />The Network Interface Device provisioned as CPE by Wave will provide 4 x 1 G port <br />hand-off on the LAN side to the client. <br />Section 4: Extension of Term. The Term of the Original Order, as amended by this <br />Second Amendment, is extended for a period of sixty (60) months from the Effective Date of this <br />Second Amendment (the "Term Extension"). Upon expiration of the Term Extension, service will <br />continue on the same terms as provided on a month-to-month basis at Provider's then current <br />rates unless terminated by either party on not less than thirty (30) days written notice to the other. <br />Section 5: Miscellaneous. Except as expressly amended by this Second Amendment, <br />the Original Order remains in full force and effect in accordance with its terms and is hereby <br />ratified and affirmed by the parties. This Second Amendment may be executed in multiple <br />counterparts, each of which shall constitute an original, and all of which shall constitute one <br />and the same instrument.
