EXHIBIT A <br />Scope of Work <br />A. Description of Services <br />Contractor shall provide closed-circuit television (CCTV) inspection and reporting on an <br />on-call basis for sanitary sewer main and lateral pipes as requested by the Public Works <br />Services Department. <br />Within 24 hours of a communication by Public Works personnel, Contractor shall <br />thoroughly inspect the sewer pipes using CCTV equipment and WinCan version <br />8.30.0.2090 inspection software or POSM. <br />Unless notified, Contractor is not responsible for pre -cleaning pipeline segment prior to <br />the CCTV inspection. Contractor's equipment shall be capable of operating in pipes that <br />are between 4 inches and 21 inches in diameter. <br />The Contractor shall be solely responsible for traffic control. <br />B. Inspection and Reporting <br />Contractor shall record the inspection digitally and recordings shall become the property <br />of the City. At the beginning of each inspection, the information of the inspection shall be <br />displayed in the following Pipeline Assessment Certification Program (PACP) format: <br />1. Surveyed by <br />2. Street <br />3. Locations Code <br />4. Direction of Inspection <br />5. Pipe Material <br />6. Pipe diameter <br />7. Length of reach to be televised <br />8. Manhole number from which the camera is traveling <br />9. Manhole number to which the camera is traveling <br />10. Pipe ID <br />11. Inspection Time/Date <br />In addition, each recording shall continuously display the following information: <br />1. City <br />2. Number of the manhole from which the camera is traveling <br />3. Number of the manhole to which the camera is traveling <br />4. Direction of inspection <br />5. Pipe diameter <br />6.
