This certified <br />copy must be certified by the Secretary or Assistant Secretary of the Corporation and the signature of the <br />Secretary or Assistant Secretary must be affirmed by a notary jurat. <br />LICENSE INFORMATION <br />Respondent shall provide on page 5 of this RFQ the number of his/her qualifying professional license procured under the <br />provisions of Article 5, Chapter 9, Division III of the Business and Professions Code of the State of California. <br />AFFIDAVIT OF NON -COLLUSION <br />Each Response must have thereon, or attached thereto, the Non -Collusion Affidavit verifying that such Response is genuine, <br />and not sham or collusive, or made in the interest or on behalf of any person not therein named; and that the Respondent <br />has not directly, or indirectly, induced or solicited any other Respondent to put in a sham Response, or any other person, <br />firm or corporation to refrain from Responding; and that the Respondent has not in any manner sought, by collusion, to <br />secure for himself an advantage over any other Respondents. Any Response not accompanied by, or which is made without <br />such affidavit, or in violation thereof, will not be considered. If the Respondent is a corporation, said affidavit shall be <br />signed by a duly authorized officer of the corporation. Any Respondent making a false affidavit may be excluded from <br />future bidding. -These terms and conditions shall apply to the full three (3) year term of this contract. <br />LIVING WAGE ORDINANCE/SERVICE CONTRACTOR WORKER RETENTION ORDINANCE <br />Unless approved for an exemption, contractors under contracts primarily for the furnishing of services to or for the City and <br />that involve an expenditure in excess of Twenty -Five Thousand Dollars ($25,000.00) and a contract term of at least three <br />(3) months, lessees and licensees of City property, and certain recipients of City financial assistance, shall comply with the <br />provisions of Los Angeles Administrative Code Sections 10.37 et seq., Living Wage Ordinance (LWO) and 10.36 et seq., <br />Service Contractor Worker Retention Ordinance (SCWRO). Bidders/Responders shall refer to the Living Wage Ordinance <br />and Service Contractor Worker Retention Ordinance for further information regarding the City's requirements. <br />Bidders/Responders who believe that they meet the qualifications for one (1) of the exemptions described in the LWO List <br />of Statutory Exemptions shall apply for exemption from the Ordinance by submitting with their response the <br />Bidder/Responder's Application for Non -Coverage or Exemption (Form OCC/LW-10), the Non-Profit/One-Person <br />Contractor Certification of Exemption (Form OCC/LW-13), or the Small Business Exemption Application (Form OCC/LW- <br />26A). These exemption forms are available on the Bureau of Contract Administration website at <br />http://bca.laciiy.org index.cfm. The List of Statutory Exemptions is included in the Attachment/Appendix. (LWO/SCWRO <br />RFQ/RFQ/RFQ Language, Rev. 08/12) <br />LOS ANGELES MUNICIPAL LOBBYING ORDINANCE AND FORMS <br />In accordance with Section 48.09(H)(1) of the Municipal Lobbying Ordinance: "Any bidder for a contract, as those terms <br />are defined in Los Angeles Administrative Code Section 10.40.1, shall submit with its bid a certification, on a form <br />prescribed by the City Ethics Commission, that the bidder acknowledges and agrees to comply with the disclosure <br />requirements and prohibitions established in the Los Angeles Municipal Lobbying Ordinance if the bidder qualifies as a <br />lobbying entity under Section 48.02 of this article. The exemptions contained in Section 48.03 of this article and <br />Los Angeles Administrative Code Section 10.40.4 shall not apply to this subsection." <br />The Bidder Certification CEC Form 50 is the aforementioned certification form prescribed by the City Ethics Commission. <br />In addition to CEC Form 50, CEC Form 55 (Prohibited Contributors form) must be submitted.
