6.D. - Page 35 of 170 <br />Request for Qualifications: <br />General Park Building Construction, Retrofit, Maintenance, and/or Repairs <br />The above forms shall be submitted to: <br />Department of Public Works Bureau of Contract Administration <br />Office of Contract Compliance <br />1149 South Broadway, Suite 300 <br />Los Angeles, CA 90015 <br />Fax: (213) 847-2777 <br />The Office of Contract Compliance may be reached at (213) 847-1922. <br />CONTRACTOR PARTICIPATION <br />The Prime Contractor shall perform one hundred percent (100%) of the total project awarded as it relates to general park <br />building construction, retrofit, maintenance and/or repairs unless otherwise authorized by the Contract Administrator. <br />Subcontracting may be allowed on a case by case basis. <br />COMMENCEMENT AND COMPLETION OF WORK <br />The work must be completed within the allowed number of days as specified in each individual project's specifications. <br />RAP will determine the number of days required to complete each project. The Contractor will be responsible for <br />completing the project's scope of work within the required project schedule. Counting of calendar days will commence on <br />the actual date work begins. It is the Contractor's responsibility to request extensions to the contract completion date in <br />writing, and RAP will determine whether an extension is justified and appropriate. <br />SAFETY ORDERS <br />The Contractor will be responsible for all safety requirements and certifications in accordance with CAL -OSHA rules and <br />regulations. It will be the Contractor's responsibility to assess the work location and implement safety controls and <br />procedures, as appropriate, which are compliant with Title 8 of the California Code of Regulations. All projects will be <br />awarded to the Contractor as a "Single Employer" environment in accordance with CAL -OSHA classifications. The <br />Contractor will be responsible and have full control over all activities in relation to the scope of work and/or services detailed <br />in this RFQ and the resultant as -needed contract, as well as any safety requirements thereof. <br />If required, the Contractor shall complete the Competent Person Trench/Excavation Certification Form provided by the <br />Bureau of Contract Administration before the end of the first (1st) day of work and prior to any workers entering a trench <br />or excavation, if applicable. <br />CALIFORNIA STATE BILL (SB) 854 <br />SB 854 took effect in the beginning 2015. All Responders responding to this RFQ must be in compliance with SB 854. <br />Among other requirements of this law, all Responders, Contractors and Sub -contractors bidding on public works projects <br />must be register with the state and pay the required annual fee to the Department of Industrial Relations (DIR). All <br />Responders, Contractors and Sub -contractors must be in compliance with SB 854 and remain current during the terms of <br />this contract. If awarded Contractor and/or Sub -contractor does not remain current with SB 854, their contract with the City <br />of Los Angeles is subject to cancellation.
