9.A. - Page 5 of 8 <br />Attachment 2 <br />BRIEF DESCRIPTION OF AGENCY FUNDING REQUESTS <br />HSFA APPLICANTS FY 2019-2020 <br />EI Centro de Libertad — Adult Substance Use Disorder Intervention <br />Program/Redwood City <br />The Adult Substance Use Disorder Intervention Program consists of outpatient <br />treatment and community outreach. Bilingual outpatient treatment includes <br />individual and group counseling, co-occurring disorder groups, trauma -informed <br />treatment, relapse prevention & life skills workshops, alcohol and drug testing, <br />and alcohol and other drug assessments for Child Protective Services. <br />Community Outreach consists of Motivational Education and Client Assessment <br />and Navigation for residents of local shelters and a bilingual parent education <br />program. Funding Requested: $15,000. Funding Recommended: $10,000. <br />Fresh Lifelines for Youth, Inc (FLY) — Law and Leadership Program for <br />Redwood City Youth. FLY's mission is to prevent juvenile crime and <br />incarceration through legal education, mentoring, and leadership training. FLY <br />will provide its Law Program to 30 at -risk Redwood City high schoolers and its <br />Leadership Program to a minimum of 4 moderate/high risk Redwood City youth. <br />Youth learn about the consequences of criminal behavior and develop the life <br />skills to say no to crime and build healthier lives. The Program is an innovative <br />law education course taught in 2 -hour sessions over 12 weeks by volunteer law <br />and university students, community professionals and FLY staff. Funding <br />Requested. $15,000. Funding Recommended. $0. <br />Health Connected — Redwood City Sexual Health Education Project. <br />Health Connected provides research -based, medically -reviewed sexual health <br />education courses to approximately 2,500 51h -121h grade students in Redwood <br />City, including some of the most underserved student populations — students in <br />special education classrooms and students enrolled in the local continuation high <br />school. Funding Requested: $20,000. Funding Recommended: $0. <br />International Institute of the Bay Area (IIBA) — Supporting Redwood City <br />Immigrant Families. I I BA's 'Supporting Redwood City Immigrant Families' <br />project provides the legal information, advice and assistance that low-income <br />immigrant families need in order to reduce fear, make educated decisions about <br />their future, and to fully integrate into the community. Funding Requested: <br />$10,000. Funding Recommended: $10,000. <br />Legal Aid Society of San Mateo County — Homesavers: Preserving <br />Affordable Housing in our Community. Legal Aid's Homesavers program <br />works to bring fairness to the county's housing courts, helps to keep low-income <br />tenants in their affordable homes, and can mitigate the effects of a potential <br />eviction. Legal Aid does this by providing legal advice and services to about 90 <br />households — including nearly 20 Redwood City residents - each month at three <br />1 <br />269 <br />
