DocuSign Envelope ID: 1976C434-096C-4F2E-A616-F74F920152DE <br />Exhibit A — Fair Oaks Core Service Program Resources List <br />Core Service Agencies Program Resources Grant <br />Grantee provides Core Services for the San Mateo County community. The Core Service <br />Agencies work in close collaboration with the Human Services Agency of San Mateo County <br />(HSA) to provide low-income residents with basic emergency and support services to stabilize <br />their living situation. The Core Service Agencies provide services, referrals, information on <br />housing resources and function as the access points for coordinated entry for homeless <br />services <br />The purpose of this grant is to assist Core Service Agencies in purchasing program resources <br />needed to ensure continuation of and enhancement of community services. <br />The County has approved grant funding for the purchase of supplies and items as shown in <br />Grant Authorized Purchases below as agreed upon by the County and the Grantee. <br />Reimbursement to Grantee is subject to the terms and conditions of this grant agreement. <br />Grantee will analyze prices and identify the most cost-effective solutions and vendors to <br />purchase supplies and items listed below. Grantee will purchase, maintain, and utilize supplies <br />and items to provide Core Services. <br />Grant Authorized Purchases <br />1. Client chairs <br />2. Desk chairs for staff <br />3. Network capable printer/scanner <br />4. Lockable, heavy duty file cabinets <br />5. Software licenses that allow for encrypting of data <br />6. IT costs related to personally identifiable information policy compliance (including IT staff <br />time) <br />The items and quantities listed above may be adjusted or replaced as agreed upon by both <br />parties and approved by the County in writing, so long as the cost does not exceed the total <br />claimable amount of the grant agreement. <br />(End — Exhibit A) <br />Template Version August 26. 2016 <br />Page <br />
