<br />6.1D <br />Page 45 <br /> <br />18.8 Maternity Leave of Absence Without Pay <br /> <br />18.8.1 <br /> <br />18.8.2 <br /> <br />18.8.3 <br /> <br />Maternity leave of absence without payor benefits shall be granted <br />upon request to non-disabled probationary and permanent female <br />employees for that period of time requested, up to one (1) year. <br /> <br />Maternity leave shall be granted when the following conditions have <br />been met: <br /> <br />18.8.2.1 <br /> <br />The employee shall notify her immediate supervisor <br />in writing, accompanied by her physician's certificate <br />of pregnancy, as soon as possible after pregnancy <br />has definitely been determined, but no later than <br />ninety (90) days prior to the tentative date on which <br />the leave is to begin. Such notice shall include the <br />tentative dates on which the leave shall begin and <br />end. <br /> <br />18.8.2.2 <br /> <br />Within thirty (30) days of the beginning of the <br />maternity leave, the employee shall submit to the <br />Human Resources Director the specific date she <br />intends to begin the leave, accompanied by her <br />physician's written statement attesting to the <br />employee's ability to continue performing the full <br />schedule of her duties and responsibilities. She shall <br />continue on active duty until the specific date, <br />providing she performs the full duties and <br />responsibilities of her position and furnishes <br />additional health statements form her physician upon <br />reasonable request. <br /> <br />18.8.2.3 <br /> <br />Prior to the establishment of a specific date for <br />return to duty, the employee shall submit to the <br />Human Resources Director a notice of intention to <br />return to duty, accompanied by her physician's <br />statement certifying that the employee is medically <br />qualified to assume regular duties and <br />responsibilities. <br /> <br />18.8.2.4 <br /> <br />The Human Resources Director or his/her designee <br />may designate the specific beginning and ending <br />dates within the pay period requested by the <br />employee to meet the needs of the employee and <br />the City. <br /> <br />The employee on leave shall be returned to her original position, or <br />if that position is not in existence, to an equivalent position within <br />her classification. <br /> <br />38 <br />
