<br />6.1C <br />Page 24 <br /> <br />9.6.2.3 <br /> <br />Prior to the establishment of a specific date for return to duty, the <br />employee shall submit to the Human Resources Director a notice of <br />intention to return to duty, accompanied by her physician's statement <br />certifYing that the employee is medically qualified to assume regular <br />duties and responsibilities. <br /> <br />9.6.2.4 <br /> <br />The Human Resources Director or his/her designee may designate <br />the specific beginning and ending dates within the pay period <br />requested by the employee to meet the needs of the employee and <br />the City. <br /> <br />9.6.3 The employee on leave shall be returned to an equivalent position within her <br />classification. <br /> <br />9.6.4 A maternity leave, absent physical disability, is granted without pay for the <br />duration of the leave. The employee may elect to continue medical and dental <br />insurance coverage for up to one (1) year during that leave at her own expense. <br /> <br />9.7 Leave for Preenancv Disabilitv <br /> <br />9.7.1 Employees who are working are entitled to use personal illness and injury <br />leave for disabilities caused or contributed to by pregnancy, miscarriage, <br />childbirth, and recovery therefrom on the same tenns and conditions governing <br />leaves of absence for other illness or medical disability. Such leave shall not <br />be used for child care, child rearing, or preparation for childbearing, but shall <br />be limited to those disabilities as set forth above. The length of such disability <br />leave, including the date on which the leave shall commence and the date on <br />which the duties are to be resumed, shall be determined by the employee and <br />the employee's physician; however, the Human Resources Director may <br />require a verification of the extent of disability through a physical examination <br />of the employee by a physician appointed by the City at City expense. <br /> <br />9.7.2 Employees are entitled to leave without payor other benefits for disabilities <br />because of pregnancy, miscarriage, childbirth, or recovery therefrom when sick <br />leave has been exhausted. The date on which the employee shall resume <br />duties shall be determined by the employee on leave and the employee's <br />physician; however, the Human Resources Director may require a verification <br />of the extent of disability through a physical examination of the employee by a <br />physician appointed by the City. <br /> <br />9.7.3 The employee on leave shall be returned to an equivalent position within her <br />classification. <br /> <br />9.8 Funeral Leave <br /> <br />9.8.1 In the event of a death in the immediate family of an employee of the City, that <br />employee shall, upon request, be granted such time offwith pay as is necessary <br />to make arrangements for the funeral and attend same, not to exceed 40 <br /> <br />18 <br />
