<br />EXHIBIT - A <br /> <br />(707) 290-5612 - Cell # <br /> <br />.riGT- <br />....~ SCHEDULED PUMP PREVENTATIVE MAINTENANCE AGREEMENT <br /> <br />SCOPE <br /> <br />A scheduled preventative maintenance agreement that will consist of a complete 21 point mechanical and <br />electrical inspection of each ITT Flygt pump listed below. This inspection will be on a _12_ month basis <br />followed by a complete report of the findings. The inspection is based upon agreement of both parties <br />involved in the services and terms stated in detail below. <br /> <br />TERMS <br /> <br />This agreement includes all labor and oil for each of the 21 points of the inspection listed on page 2. Any <br />additional repairs not stated on the 21-point check list required during the inspection would be performed <br />only by written authorization. The repairs will be at no charge if covered by warranty* or standard part and <br />labor rates if not covered by warranty and will be begin immediately. This agreement excludes damage by <br />storms, other acts of nature, etc. <br /> <br />ITT Flygt assumes no liability for loss of use or for any consequential damage with respect to the operation of <br />the pump(s), equipment or accessories covered by this agreement. <br /> <br />The owner's responsibility will be to have the following at the job site when the ITT Flygt service technician <br />arrives: <br /> <br />A. Water supply for washing pump(s) <br />B. 120 Volt electric supply <br />C. Mechanic to assist in the inspection and learn proper techniques <br />D. Electrician (if control not supplied by ITT Flygt) <br />E.
