<br />public health and welfare, nor unless such dog has attached <br /> <br />to its collar the vaccination tag required under Section 5.24.1 <br /> <br />hereof. <br /> <br />IIsection 5.24.1 The canine anti-rabies vaccination <br /> <br />required hereunder shall be administered by a duly licensed <br /> <br />veterinarian or such other person authorized to administer <br /> <br />such vaccination at a vaccination clinic approved by the <br />health officer. Such vaccination shall be ~dministered at <br /> <br />intervals of time specified by the health officer, but in <br /> <br />no event more often than once a year. Every veterinarian <br /> <br />or other authorized individual shall, after vaccinating a <br /> <br />dog with an anti-rabies vaccine, certify that such vaccination <br /> <br />has taken place by issuing a certificate to the owner or <br /> <br />custodian of such dog, and shall affix a vaccination identi- <br /> <br />fication tag to the collar of such dog evidencing the fact <br /> <br />of such vaccination. The certificate shall include the <br /> <br />following information: <br /> <br />11(1) Owner's or custodian's full name, address <br />and telephone number. <br /> <br />"(2) vaccination tag number. <br /> <br />II (3) Breed, age, sex and color or markings of <br />the vaccinated dog. <br /> <br />"(4) A statement certifying that said dog was <br />vaccinated against rabies with either: <br /> <br />U(i) Live virus vaccines of the following types: <br /> <br />"a. Chick embryo origin; or <br /> <br />lib.
