EXHIBIT B <br /> <br /> Youth and Family Assistance <br /> Daybreak Shelter <br /> <br /> CONTRACT OBJECTIVES <br /> <br /> FISCAL YEAR 1999/2000 <br /> <br /> GOALS # OF PERSONS I COMPLETION <br /> ASSISTED I DATE <br /> <br /> Primary Quantitative Contract Goals <br />Five (5) residents of Redwood City will be 5 persons June 30, 2000 <br />admitted to the Daybreak Shelter <br />component <br /> <br />Two (2) residents of Redwood City will be 2 persons June 30, 2000 <br />admitted to the Shared Housing and/or <br />Aftercare components. <br /> <br />Eighty percent (80%) of the youth with 10 persons June 30, 2000 <br />individual case plans will secure and <br />maintain employment. <br /> <br />Thirty percent (30%) of the youth with 4 persons June 30, 2000 <br />individual case plans will complete a job <br />training program or secure a high school <br />equivalency degree. <br /> <br />Sixty-five percent (65%) of the youth with 8 persons June 30, 2000 <br />individual case plans will transition to a <br />positive living arrangement. <br /> Other Secondary Collaborative Actiom Outreach and Emphasis to be Conducted <br />None <br /> <br /> <br />
