0 R I G I N A L 2/24/97 <br /> RESOLUTION NO. 13007 <br /> <br /> RESOLUTION APPROVING AND AUTHORIZING EXECUTION <br /> OF AMENDMENT TO AGREEMENT FOR EMPLOYEE <br /> ASSISTANCE PROGRAM SERVICES <br /> <br /> (MANAGED HEALTH NETWORK) <br /> <br /> WHEREAS, the City of Redwood City and Managed Health Network <br />(formerly Occupational Health Services, Inc. "OHS"), entered into an agreement <br />dated July 11, 1994 (the UBasic AgreemenC) pursuant to which Managed Health <br />Network has provided a program of counseling services to employees of the City <br />of Redwood City with various personal and medical behavioral problems, in <br />order that said employees more effectively perform their employment; and <br /> <br /> WHEREAS, pursuant to paragraph VI.E., of the Basic Agreement, the <br />parties renewed and extended the term and revised the compensation <br />thereunder in accordance with Amendment thereto dated January 23, 1995; and <br /> <br /> WHEREAS, pursuant to paragraph VI.E., of the Basic Agreement, the <br />parties renewed and extended the term thereof and revised the compensation <br />and the Benefits and Services provided under Section 3 of Addendum Three <br />thereunder in accordance with a Second Amendment thereto dated February <br />23, 1996; and <br /> <br /> WHEREAS, the parties thereto desire to renew and extend the term <br />thereof and to revise said Benefits and Services provided under Section 3 of <br />Addendum Three and to add the Department of Corporations Consumer <br />Complaint toll free number under "Grievance Procedures" of the Combined <br /> <br /> Reso-140 <br /> DES:dJk <br /> <br /> <br />
