Evidence of Coverage and Disclosure Form thereunder for the period of January <br />1, 1997 through December 31, 1997; and <br /> <br /> WHEREAS, a Third Amendment to the Agreement for Employee <br />Assistance Program Services has been presented to, and reviewed by, this <br />Council; <br /> <br /> NOW, THEREFORE; <br /> <br /> BE IT RESOLVED BY THE COUNCIL OF THE CITY OF REDWOOD <br />CITY, AS FOLLOWS: <br /> <br /> That certain agreement by and between the City of Redwood City, a <br />municipal corporation of the State of California, and Managed Health Network <br />(formerly Occupational Health Services, Inc. "OHS"), a Califomia corporation <br />entitled, "Third Amendment to Agreement for Employee Assistance Program <br />Services", (form dated 02/10/97) providing personal counseling services for <br />employees of the City of Redwood City, a copy of which agreement is on file in <br />the office of the City Clerk, to which copy reference is hereby made for the full <br />particulars thereof, is hereby approved, and the City Manager and City Clerk are <br />hereby authorized and directed to execute, and attest thereto, respectively, said <br />Amendment for and on behalf of the City of Redwood City. <br /> <br /> Reso-140 <br /> DES:dJk <br /> o2/1 <br /> <br /> <br />
