The Vision Service Plan S <br /> provides for an exam every 12 rnonths, lenses every 12 months if needed, and <br /> frames every 24 mon�hs if needed. There will be no deductible for employees, <br /> but a $20.00 per person deductible will apply ta dependents each time benefits <br /> are available and wil! be paid by the employee. <br /> d. Savinq� �iause — If, pursuant tc� �r�� feu��a1 c�r 5iate iaw whic�i rriay �eeunc <br /> efFective subsequent to the effective date of this policy, the City is required to <br /> pay cantributions or taxes for hospital-medica!-surgical, dental care, prescription <br /> drug or other health benefits to be provided its emplayees Gnder such federal or <br /> state act, the City's obligation to furnish the same benefits under the hospital- <br /> medical programs shall be suspended and the contributions agreed to be paid <br /> monthly hereunder by the City shall be reduced each mor►th by the amounts <br /> which the city is required to expend during such month in the form of <br /> contributions or taxed to support said federal or s�ate health plan. <br /> If, as a result of such law, the level of benefits provided by such law fflr any <br /> graup of employees, or fiheir dependents, is lower in certain categories af <br /> services than that provided under the existing major plan, the City shall, to the <br /> extent practical, provide a pfan of benefits supplementary to the federal or state <br /> benefits so as to make benefits in each category of coverage as nearly <br /> c��n�ara�iG as p�ss���� t� th� b�nef;t� �,rovid°� u^uer t"° existing major plan. <br /> The City need only exp�nd for this purpose the actual amount required to <br /> achieve parity between the benefits provided under any federal or state plan as <br /> supplemented in the manner hereinabove described. In no event shall the City <br /> be required to expend for such purposes an amount which when added to the <br /> conrributions or taxes required of th� City unde� the federal or state act, sraii <br /> exceed the amounts paid at the time such legislation becomes effective. <br /> If the benefts provided under the federal or state act exceed the benefits <br /> provided hereunder in each category af coverage, the City shali be under no <br /> further obligation to make any contributian. <br /> e. t�ife Insurance —The City shall provide "basic" life insurance coverage af <br /> $3,OOQ to all members of the Executive Managementi Group.
