In the event of the death of an employee, such payments <br />shall be made to the designated beneficiary filed with the <br />Director of Finance, or, in the event no designated <br />beneficiary has been chosen, the beneficiary listed in the <br />unit member's life insurance policy will receive payment of <br />such unused sick leave as provided under the provisions <br />for the employee who elects to receive compensation in <br />lieu of sick leave credits. Employees discharged shall not <br />be eligible for payment of unused sick leave. <br />18.1.5.4 Unused accumulated sick leave at the time of retirement, <br />for which the employee receives no compensation, shall <br />be converted to additional service credit at the rate of <br />0.004 years of service for each day, i.e., 250 days of sick <br />leave for one additional year of service credit. Local safety <br />member benefits subject to a maximum of 90% of the <br />average final compensation will not be increased beyond <br />that limit for unused sick leave credit. <br />18.1.6 Procedures for Requesting and Approving Sick Leave <br />Employees are responsible for notifying the Battalion Chiefs office <br />of any inability to report to duty prior to the commencement of their <br />work shift. Shift employees shall notify the Battalion Chiefs office <br />prior to 0700 hours on the day the employee is to be absent and <br />shall enter the leave into the Department's automated scheduling <br />software. Non -shift employees shall notify their immediate <br />supervisor by 0815 hours. Notification shall include the reason and <br />the possible duration of the absence. <br />Employees are responsible for keeping the Department informed of <br />their continuing condition and probable date of return to work. <br />For absences in excess of three (3) or more consecutive work days <br />or duty shifts, the Fire Chief may require a physician's statement <br />from an employee who applies for additional sick leave, or whatever <br />investigation into the circumstances which appear warranted, before <br />taking action on the request. <br />18.2 Limited Light Duty Work For An Injured Employee <br />18.2.1 An employee who is receiving medical attention for a reported <br />industrial injury, and who is determined to be temporarily disabled, <br />is required to return to the department after initial treatment with a <br />medical report indicating what, if any, limited duty the employee can <br />perform.
