Dedication of Public Service Easement to facilitate construction of Silicon <br />Valley Clean Water Gravity Pipeline through the City owned parcel on Radio <br />Road <br />Recommendation: <br />By motion, approve and authorize the City Manager to execute the Public <br />Service Easement. <br />CEQA: <br />This is not a project under California Environmental Quality Act (CEQA) <br />6.D. Final map and associated agreements for the development at 612 Jefferson by <br />Habitat for Humanity <br />Recommendation: <br />1. By motion, approve the Final Map for 612 Jefferson Avenue. <br />2. By motion, approve and authorize the City Manager to execute an <br />Improvement Agreement with Habitat for Humanity Greater San Francisco. <br />3. By motion, approve and authorize the City Manager to execute a Landscape <br />Maintenance Agreement with Habitat for Humanity Greater San Francisco. <br />4. By motion, approve and authorize the City Manager to execute amendments <br />to the Affordability Agreement and Affordability Covenant with Habitat for <br />Humanity Greater San Francisco. <br />5. By motion, authorize the City Manager to make any minor, clarifying and <br />conforming changes to the agreements specified above as approved by the City <br />Attorney. <br />CEQA: <br />Project consistent with Previous Plan EIR <br />6.E. Amendment No. 3 to professional services agreement with AECOM Technical <br />Services, Inc. (AECOM) for the 101/84 (Woodside Road) Interchange Project <br />Plans, Specifications and Estimate (PS&E) and Right of Way Support (R/W <br />Support) phase <br />Recommendation: <br />By motion, approve and authorize the City Manager to execute Amendment No. <br />3 to the Agreement with AECOM in the amount not to exceed $504,761 for <br />additional services pertaining to custom structure aesthetic design, right of way <br />support and field surveys, and extend the term of Agreement to September 30, <br />2020; and authorize the City Manager to further amend the Agreement in an <br />amount not to exceed $125,000 to cover supplemental additional services (if <br />needed). <br />Page 3of8 <br />
