REV: 07-12-22 RL <br />Consultant will work with the City to develop sustainable funding streams to offset City financial <br />contributions in fiscal year 2020-2021 and beyond, while maintaining the same level of service <br />in the City throughout. <br />Consultant will meet at least once during each fiscal year of this Agreement with the Public <br />Works Director and Homeless Services Manager for a performance review. <br />Consultant will build up to a standing Team of at least 20 individuals and serve 60 individuals <br />as team members in the contract year. Consultant's staffing will be comprised of three full-time <br />individuals and four part-time individuals. A Project Manager will manage Consultant staff and <br />Team Members, as well as all operations, fundraising efforts, facilitating the weekly Team <br />meeting and be responsible for all community relations. The Project Manager will act as the <br />direct contact for Redwood City and all project partners. A Case Manager and Employment <br />Specialist will provide resources to Team Members, provide one-on-one case management <br />sessions, provide employment services, advocate for Team Members and build partnerships <br />with service providers, employer and landlord to place Team Members. Four part-time <br />Drivers/Outreach Workers will provide services to address illegal dumping and encampment <br />waste services with Team Members. <br />Consultant will report on the following outcomes on a monthly basis: <br />§ Team size and individuals served <br />§ Team Members graduated into employment <br />§ Team Members moved into permanent housing <br />§ Debris removed in cubic feet, including needles removed (Police/Public Works will <br />partner on training and removal) <br />§ Volunteer hours served <br />§ Team Member survey results collected annually reflecting self-esteem, motivation, and <br />hope as well as positive outcomes re health, mental health, substance abuse treatment, <br />criminal justice system interactions <br />During the Term of the Contract, Consultant is committed to fulfilling the following objectives: <br /># of unique Team Members served: 60 <br /># of unique non-Team Members served: 40 <br /># of barriers removed: 200 <br /># of individuals seeking housing: 40 <br /># of individuals housed: 10 <br />% of housing-seeking individuals housed: 25% <br />% of individuals actively working with Case Manager: 60% <br /># of individuals seeking employment: 40 <br /># of individuals employed: 20 <br />% of employment-seeking individuals employed: 50% <br /># of employed individuals who keep their job over 90 days: 10 <br />% of employed individuals who keep their job over 90 days: 50% <br />% of individuals actively working with Employment Specialist: 60% <br /># of gallons of Trash Removed: 450,000 <br />Response to Illegal Dumping Requests in My RWC within 72 hours <br />Pro-active Illegal Dumping monitoring and clean-up 5 days per week <br />Outreach re Encampment Waste Services at all homeless encampments at least 4 times <br />a year <br />ATTY/AGR.2022/Amend.
