REV: 11-03-2021 SK <br />Exhibit “A” <br />SCOPE OF SERVICES AND FEES <br />Task 0: Project Management <br />Consultant shall be available throughout the life of the project to coordinate with City staff via <br />email or phone to discuss deliverables and provide updates as the project progresses. <br />Consultant’s Project Manager shall be responsible for managing the project, developing follow- <br />up notes and action items from meetings, maintaining the project schedule, overseeing the <br />budget, preparing and submitting monthly invoices and progress reports, and being a resource <br />for City staff. <br />TASK 0 DELIVERABLES <br />Materials for and facilitation of check-in calls as needed <br />Monthly progress reports and invoicing <br />Task 1: Community Outreach <br />TASK 1.1: POST-INSTALLATION COMMUNITY SURVEY <br />Consultant shall develop an online survey in English and Spanish to solicit input on the Vera <br />Avenue Bicycle Boulevard project after quick-build implementation. The survey results will be <br />used to identify the overall community reaction and any needed modification. <br />Consultant shall work with the City to determine the questions. The topics for survey questions <br />may include: <br />Reasons for using Vera Avenue (e.g., commuting, passing through, recreation, etc.) <br />Frequency of use <br />Travel modes used on Vera Avenue <br />Comments on implemented improvements and materials used <br />Comments on street operations and intuitiveness of design elements <br />Level of comfort and perceived safety <br />Recommended changes <br />Consultant shall submit a draft survey (in English) for review by the City. Consultant shall revise <br />the survey based on one round of consolidated comments from the City to develop the final <br />content in English and Spanish. The survey shall be designed in hard copy format to be <br />distributed to the corridor residents. <br />COMMUNITY SURVEY ASSUMPTIONS <br />Consultant shall upload the online survey using Alchemer (SurveyGizmo) <br />The survey link can be included on future community outreach materials such as flyers <br />and postcards. The survey will remain live for a few months to gather input after people <br />have used the bike boulevard for a while. <br />City is responsible for publicizing the online survey as well as for printing, distributing, <br />and paying for return postage of hard copy surveys. <br />ATTY/AGR.2021 - Amend No. 1/Toole Design Inc.
