6.D. - Page 28 of 65 <br />Attachment C <br />PLEASE SUBMIT ALL PAGES OF THE RFP, AS 1T RELATES TO THIS PROPOSAL, PLEASE TURN IN ALL <br />PAGFS <br />ATTACHMENT A- PROPOSAL PRICING FORM <br />Price must be complete and include tax, delivery, labor, and other charges. Prices remain firm for one- <br />year after award of contract to Successful Proposer. <br />If the unit price and the total amount named by a Proposer for any item do not agree, it will be assumed <br />that the error was made in computing the total amount and the unit price alone will be considered as <br />representing the Proposer's intention. <br />All proposal items, including lump sums, unit prices, and additive alternates, must be filled in completely. <br />Quote in numerals only, unless words are specifically requested. <br />The Proposer shall provide Elevator Maintenance Services for all elevators and lifts owned by the City <br />of Redwood City, California. The project area covers approximately nineteen (19) square miles of <br />urbanized neighborhoods and commercial districts of the City. The City owns twelve (12) hydraulic <br />elevators, three (3) wheelchair lifts, and one (1) material lift. <br />A. Contractor Competence Requirements <br />During the Term of the Agreement, the Contractor shall satisfy and maintain the following criteria: <br />1. The Contractor shall have a current license to perform the work in conformance with the <br />provisions of the State Business and Professions Code. <br />2. The Contractor shall have the requisite skilled and experienced elevator maintenance staff <br />under its direct employment and supervision. <br />3. The Contractor shall certify that a member of its supervisory personnel, who is regularly <br />engaged in inspection and repair supervision, will visit each listed elevator at least semi- <br />annually to observe the quality of maintenance and to make certain that the quality of <br />maintenance meets the specified standards. The supervisor shall report his/her findings in <br />writing to the City's Facilities Maintenance Supervisor. <br />4. The Contractor shall have the necessary access to repair parts so they can be obtained within a <br />maximum of forty-eight (48) hours, in order to minimize elevator downtime. <br />5. The Contractor shall have a Call Center that is available 24 hours per day, 365 days per year, <br />which can dispatch technicians to the site within the specified time allowed. <br />B. Preventative Maintenance Requirements <br />1.
