serve as a juror shall not have his regular starting or quitting time changed as a <br />result of being called for jury service. <br />9.6 Maternity Leave of Absence Without Pgy <br />9.6.1 Maternity leave of absence without pay or benefits shall be granted upon <br />request to non -disabled probationary and permanent female employees for <br />that period of time requested up to one (1) year. <br />9.6.2 Maternity leave shall be granted when the following conditions have been <br />met: <br />9.6.2.1 The employee shall notify her immediate supervisor in writing <br />accompanied by her physician's certificate of pregnancy as <br />soon as possible after pregnancy has definitely been <br />determined, but no later than ninety (90) days prior to the <br />tentative date on which the leave is to begin. Such notice shall <br />include the tentative dates on which the leave shall begin and <br />end. <br />9.6.2.2 Within thirty (30) days of the beginning of the maternity leave, <br />the employee shall submit to the Personnel Officer the specific <br />date she intends to begin the leave, accompanied by her <br />physician's written statement attesting to the worker's ability to <br />continue performing the full schedule of duties and <br />responsibilities. She shall continue her regular active duty until <br />the specific date providing she is able to perform the full duties <br />and responsibilities of her position and furnishes additional <br />health statements from her physician upon reasonable request. <br />In the event the employee is unable to perform the full duties <br />and responsibilities of her position, she shall be assigned to <br />light duty until the specific leave date, and shall continue to <br />furnish additional health statements from her physician upon <br />reasonable request. <br />9.6.2.3 Prior to the establishment of a specific date for return to duty, <br />the employee shall submit to the Personnel Officer a notice of <br />intention to return to duty, accompanied by her physician's <br />statement certifying that the employee is medically qualified to <br />assume regular duties and responsibilities. <br />9.6.2.4 The Personnel Officer or his/her designee may designate the <br />specific beginning and ending dates within the pay period <br />requested by the employee to meet the needs of the employee <br />and the City. <br />9.6.3 The employee on leave shall be returned to an equivalent position within <br />her classification. <br />18 <br />
