<br />6.2A <br /> <br />Attachment 3 - Project Timeline Page 15 <br /> <br />Project Timeline <br /> <br />Due to the nature of the service being provided, the Fair Oaks Community Center <br />Information and Referral Program would implement the Early Entry Shelter Program on <br />an ongoing basis effective at the commencement of the CDBG contract for FY 08-09, <br />July 1, 2008. <br /> <br />Specific work tasks will include: <br />. Client assessment and screening for Early Entry Shelter Program (EESP) <br />eligibility. Eligibility includes a willingness to go to either ofthe two primary <br />single shelters in San Mateo County and eligibility to access either. <br />. Assisting client to access other specialized assessments/services where this <br />would be useful in best detennining how to assist the client in ending their <br />homelessness (ie; detox program, alcohol and drug assessment, or mental <br />health assessment). <br />. Providing initial case management services while EESP client awaits entry <br />into the shelter - including initial shelter intake, client emergency and medical <br />background, beghming TB test process, assistance with accessing necessary <br />medical, employment or benefits services, and depending on the length of the <br />wait to enter shelter - beginning to develop a case plan which can then be <br />continued once the person enters the shelter. <br />. Coordination with shelter providers and other specialized service providers. <br />. Ongoing training of local police regarding homeless services available <br />through Fair Oaks Community Center. <br />. Ongoing outreach regarding homeless services available at the Fair Oaks <br />Community Center. <br /> <br />For purposes of reimbursement, the City of Redwood City will report on regular basis - <br />as required by the CDBG program administration. <br /> <br />Accomplishments and Maior Milestones Targetted for FY 08-09: <br />)> Maintain increased access to shelter for homeless individuals seeking shelter. <br />)> Increased number of individuals seeking shelter who access other non-shelter <br />services to end their homelessness. <br />>- Stronger connection to mental health services for homeless individuals for whom <br />their mental health issues presents barriers to their ability to access and benefit <br />from the shelter system. <br />'r Wrap-around case management for higher risk homeless individuals - beyond the <br />initial case management services already provided and in partnership with the <br />shelter staff. <br />~ Follow-up on outcomes. for clients served througll EESP as cOlnpared to clients <br />entering shelter without EESP. <br /> <br />Accomplislunents and Maior Milestones for FY 07-08 have included: <br />)> Launching of the Early Entry Shelter Pilot Program in April 2007 and securing of <br />funding for the continuation of the pilot program through June 2008. <br />
