6.2A Attachment 3 - Project Timeline Page 15 Project Timeline Due to the nature of the service being provided, the Fair Oaks Community Center Information and Referral Program would implement the Early Entry Shelter Program on an ongoing basis effective at the commencement of the CDBG contract for FY 08-09, July 1, 2008. Specific work tasks will include: . Client assessment and screening for Early Entry Shelter Program (EESP) eligibility. Eligibility includes a willingness to go to either ofthe two primary single shelters in San Mateo County and eligibility to access either. . Assisting client to access other specialized assessments/services where this would be useful in best detennining how to assist the client in ending their homelessness (ie; detox program, alcohol and drug assessment, or mental health assessment). . Providing initial case management services while EESP client awaits entry into the shelter - including initial shelter intake, client emergency and medical background, beghming TB test process, assistance with accessing necessary medical, employment or benefits services, and depending on the length of the wait to enter shelter - beginning to develop a case plan which can then be continued once the person enters the shelter. . Coordination with shelter providers and other specialized service providers. . Ongoing training of local police regarding homeless services available through Fair Oaks Community Center. . Ongoing outreach regarding homeless services available at the Fair Oaks Community Center. For purposes of reimbursement, the City of Redwood City will report on regular basis - as required by the CDBG program administration. Accomplishments and Maior Milestones Targetted for FY 08-09: )> Maintain increased access to shelter for homeless individuals seeking shelter. )> Increased number of individuals seeking shelter who access other non-shelter services to end their homelessness. >- Stronger connection to mental health services for homeless individuals for whom their mental health issues presents barriers to their ability to access and benefit from the shelter system. 'r Wrap-around case management for higher risk homeless individuals - beyond the initial case management services already provided and in partnership with the shelter staff. ~ Follow-up on outcomes. for clients served througll EESP as cOlnpared to clients entering shelter without EESP. Accomplislunents and Maior Milestones for FY 07-08 have included: )> Launching of the Early Entry Shelter Pilot Program in April 2007 and securing of funding for the continuation of the pilot program through June 2008.
